Nomination Form - 2011
Country Visions Cooperative Nominating Committee will be
meeting in a few weeks to find candidates willing to put their name on the
ballot for the next election to our Board of Directors. If you are interested in learning more
about how our Co-op is run and would like to share your ideas for our
future, please fill out this form and return it to our office by January 2, 2011.
To qualify to be a member of the Board you must be a
producer who has spent at least $5,000 in our fiscal year. Each nomination form must be signed by
two voting members in good standing.
Signatures can be obtained from a nomination committee member, a
voting member of the Co-op, and/or the nominee themselves.
Our 2009/2010 Annual Meeting will be held on February 10, 2011 at the Club
Bil-Mar in Manitowoc. All nominations must be submitted to the
Country Visions Cooperative office in Reedsville by January 2, 2011.
